# How to choose which codes are saved into the patient record?

## How to choose which SNOMED codes are saved from C the Signs into the patient records following a Risk Assessment or referral

After completing a C the Signs Risk Assessment or referral, you can choose which codes are saved into the patient's record.

- C the Signs will automatically populate the relevant codes and these will be shown in green at the bottom of the screen.

- If you wish to deselect any of the codes you can click on the 'cross' symbol

- If you wish to reselect any of the codes you have deselected, just click back on the code.
