Why has the 12-month CCR been ticked off when only the 3-month review has been done?
- Usually, this is because the 12-month CCR code has been added in error to the notes when the clinician has performed the 3-month review in the electronic health record. This is common because the 12-month code is the words ‘cancer care review’

How do I untick a CCR that has been added in error?
In order to reset the CCR to unticked on C the Signs, you will first need to remove the code from the patient's notes.
Then you will need to remove the patient from the Cancer Confirmed Board using the 'Remove.. Patient from current board' button as shown in the image below (please make a note of the patient information before doing this, e.g. diagnosis type, date of diagnosis):
You can then re-add the patient to the dashboard using the 'Add Patient' button as shown below:
